What happens during the assessment process?

If you are interested in NHS continuing healthcare funding you may need to be assessed. If you  would like to know more about the assessment process then hopefully this article can provide with you the information you need. 

The process begins with an initial screening to see if you are eligible for funding. This is usually carried out in hospital or in the home by a doctor, nurse, social worker or other healthcare professional. 

They will assess a variety of different things including;  

  • Behaviour  
  • Communication 
  • Cognition 
  • Mobility  
  • Psychological and emotional needs  
  • Nutrition 
  • Breathing 
  • Symptom control 
  • And other significant care needs. 

After this screening it is then on to the assessment. 

The care assessment process can be pretty complex but do not be put off by this. Just remember, after the assessment you may get the help that you need to manage your heath. 

The assessment will be carried out by two or more health and social care professionals who are involved in your care.

This team will use much the same check-list as above but they will go into a lot more detail. They will mark each of your care needs as; low, moderate, high, severe or priority.

If you have one priority need or two serve needs you should qualify for funding. If you have one serve need and a number of other high or moderate needs then you may still qualify.
