What happens after the assessment?

After your care needs assessment has been carried out, your local authority will then decide what care services it can provide or arrange for you. 

They may also give you the option of arranging these services for yourself through direct payments. 

This decision is made by comparing your needs to a set of nationally agreed criteria which all local authorities have to use.

Agreeing a package of care

After your care needs assessment has been completed and the local authority has decided which care services they can offer you, a care package will then be formed. 

This care package is decided upon by you and the local authority. You have a decision as to which care you receive and you will get a written copy of your care plan.

The care services available to you can include;

  • Home care to help with things around the house 
  • Day centres to give you or the person you care for a break 
  •  Day care for your child  
  • Disability equipment and adaptations to your home  
  • Residential care or nursing homes  

Reviewing your care and support plan

Once your care and support plan has been agreed you have the right to ask for a review at any time. If you think that your care, wellbeing or financial circumstances have changed then you can ask the council to review your plan. 

Even if nothing has changed, the council must still review your care plan regularly, usually once a year.
